Can dogs walk again after IVDD?
Many dogs do regain useful walking after IVDD, but there is no single recovery percentage that applies to every dog. Outcome depends heavily on the severity and location of spinal cord injury, neurological status, treatment and complications.
What affects the chance of walking again?
- Whether the dog can still walk at presentation.
- Whether voluntary movement remains.
- Veterinary assessment of deep pain perception in severe cases.
- The location and degree of spinal cord compression or injury.
- Whether neurological signs are stable or progressing.
- The treatment selected and complications during recovery.
What does “deep pain positive” or “deep pain negative” mean?
In dogs with severe spinal cord injury, a veterinary neurologist may assess whether the brain still consciously perceives a strong painful stimulus from the affected limbs or tail. This is called deep pain perception and it can be an important prognostic finding.
Deep pain positive means the veterinary examination finds evidence that conscious pain perception is still present. Deep pain negative means that response cannot be demonstrated on the neurological examination. Loss of deep pain is associated with a more severe spinal cord injury and a more guarded prognosis, but it does not by itself tell an owner exactly what an individual dog will eventually be able to do.
Can a deep-pain-negative dog ever walk again?
Some dogs with very severe spinal cord injury do later regain purposeful movement. Others may remain without conscious pain perception yet develop a form of coordinated stepping often called spinal walking. Outcomes vary greatly, so this should never be presented as an expected or guaranteed result.
What is spinal walking?
Spinal walking describes functional stepping that can develop through locomotor circuits within the spinal cord even when normal communication with the brain remains severely disrupted. The movement may look different from normal walking and can vary in stability, coordination and endurance.
This distinction matters: neurological recovery and functional mobility are not always the same thing. A dog may remain neurologically impaired yet achieve meaningful independence through rehabilitation, spinal walking, mobility aids, a wheelchair or a combination of approaches.
Where do physiotherapy, slings and assisted walking fit?
Veterinary rehabilitation is commonly used after IVDD and spinal cord injury. Depending on the dog and stage of recovery, a veterinary rehabilitation professional may recommend supported standing, assisted stepping, sling-supported walking, controlled exercise, range-of-motion work or other techniques.
For dogs with persistent paralysis, some rehabilitation teams continue structured locomotor training in an effort to improve strength, coordination and functional mobility. Evidence about the ideal programme and exactly which dogs will develop spinal walking remains limited, so rehabilitation should be individualised rather than treated as a formula.
A sling should support safe movement, not force it. Ask your veterinary or rehabilitation team how much weight to support, how often to practise and whether assisted walking is appropriate at your dog’s current stage.
Recovery is more than the first few days
Nervous-system recovery can continue over weeks and months. Some dogs improve quickly; others make slower gains. Early inability to walk after surgery does not automatically mean treatment has failed, and a plateau does not always mean no further functional gains are possible.
Walking quality matters too
Owners may notice an unsteady gait, paw scuffing, crossing of the legs or weakness before movement becomes smoother. Keep videos and simple notes if your veterinary team finds them useful; changes that are difficult to notice day-to-day may become clearer over time.
What if normal walking does not return?
Some dogs can still have a good quality of life with assisted mobility, nursing support or a wheelchair when appropriate. The goal is comfort, engagement, hygiene and safe independence — not forcing every dog toward the same definition of “normal”.
One important complication to understand
Dogs with the most severe spinal cord injuries may be monitored for progressive myelomalacia, a rare but devastating complication in which spinal cord damage progresses. If your veterinary team is concerned about this, prognosis and monitoring are very different from routine rehabilitation.
