IVDD MRI for dogs: when is advanced imaging needed?

MRI can help a veterinary specialist identify the spinal lesion causing a dog's neurological signs and show the spinal cord, discs and surrounding soft tissues in detail. Whether MRI, CT or another imaging test is appropriate depends on the neurological examination, suspected lesion, urgency and the treatment decision being considered.

What MRI Can Show

MRI helps specialists see the spinal cord and affected discs in detail

MRI is widely used to identify the location and character of intervertebral disc herniation, assess spinal cord compression and injury, and help distinguish IVDD from other conditions that can cause similar neurological signs.

For acute thoracolumbar disc extrusion, the 2022 ACVIM consensus cites diagnostic sensitivity above 98.5% in the studies it reviewed. The scan is still interpreted alongside the neurological examination and clinical history; it is not a standalone treatment plan.

MRI, CT and X-rays

Which scan is used for suspected IVDD?

There is no single imaging test used in every dog. Veterinary specialists choose between MRI, CT and, less commonly, myelography or CT-myelography based on the neurological examination, suspected lesion, urgency, availability and whether surgery is being considered.

MRI

MRI shows the spinal cord, intervertebral discs and surrounding soft tissues in high detail. The 2022 ACVIM consensus reports diagnostic sensitivity above 98.5% for acute thoracolumbar disc extrusion in the studies it reviewed, and notes advantages over CT in some presentations and for differentiating extrusion from protrusion.

CT

CT can be an excellent first-line advanced imaging option in selected dogs, particularly young-to-middle-aged chondrodystrophic dogs where acute thoracolumbar disc extrusion with mineralised material is suspected. It is often faster than MRI and can be highly effective for locating mineralised extruded disc material, although it provides less direct information about the spinal cord itself.

Plain X-rays

X-rays can show bones, alignment and disc calcification, but they cannot directly show the spinal cord or reliably confirm the exact site and nature of spinal cord compression.

The aim is to identify the lesion that best explains the neurological examination and, when necessary, provide enough anatomical detail to guide treatment or surgery.

When Advanced Imaging May Be Recommended

Situations where your veterinary team may discuss MRI or CT

Neurological signs are worsening

Increasing weakness, loss of walking ability or rapidly changing neurological function can make specialist assessment and advanced imaging more urgent.

Surgery is being considered

Advanced imaging helps the specialist locate and characterise the lesion and plan an appropriate surgical approach when surgery is recommended.

The diagnosis is uncertain

Other spinal, neurological and orthopaedic conditions can resemble IVDD. MRI or CT may be needed when the cause is unclear.

Pain is severe or persistent

Severe, recurrent or persistent spinal pain despite treatment may prompt further investigation depending on the clinical examination.

Is MRI Always Needed?

Not every suspected IVDD case has an MRI immediately

Mild, stable signs

Some dogs with mild neurological signs may initially be managed conservatively without immediate advanced imaging, depending on the veterinary examination and response to treatment.

Improving with treatment

If a dog is neurologically stable and improving, the veterinary team may decide that immediate imaging would not change the current plan.

Imaging would not change treatment

A useful question is whether the result would change treatment, referral or surgical planning. Your vet can explain this in your dog's specific case.

Questions beyond the scan itself

Anaesthesia

Dogs need to remain completely still for MRI, so general anaesthesia is commonly used. The veterinary team assesses anaesthetic risk based on the individual dog's health, airway, medications and circumstances.

Referral and availability

MRI is usually performed at a referral hospital or advanced imaging centre. Availability and urgency vary by location.
